Sept 6th "Update":

There are 6 v2 stalls at the Burbank Service Center. Most recently I've seen signs that state public charging is available after 7pm. Even as recently as this year I was able to pull into an available stall with no wait.

I don't chase firsts like @Limited does but this one is pretty much as close as a Supercharger could reasonably get to my house. If I'm ever going to get a local first, it was this one.

2022-01-25: Glendale, CA - Harvey Drive (First)

The document in the original post by @MarcoRP shows what appear to be 8 Supercharger stalls.

Install looks pretty complete to me. Transformer is in, pavement sealed, lines painted, wheel stops installed, etc. Appears to be ready to go but I can confirm that it is not powered quite yet. Also pictured are the two ADA stalls that are off to the side by Fuji.

Paving equipment was arriving early this morning.
