I’ve had this problem and believe it’s from the USB drive overheating. Only happened with the little Sandisk 128MB ultra disk drives, never had an issue with using the Samsung T5 portable SSD.

I have the iPhone 12 Pro Max with the Apple MagSafe Leather case and the Tesla branded wireless pad for my Model 3 and it charges fine. The fit is tight and I have to push the phone down/under the bottom “lip” a little, but it ends up fitting and charging fine.
