I have a NYS EZ-Pass and a 2016 Model S and it works 100% of the time in and out of NY, NYC, NJ, CT, MA, etc.
The trick is that you have to mount it upside down, i.e., so the E-Z Pass lettering is upside down. Someone on here took one apart and found that the transmitter is on the right side (when right side up).
I have a similar issue but I wouldn't call it a creak, more like a vibration or rattle. I originally thought it was the fan going but then I moved the vent louvers as you did and it goes away.

Sure, here it is - mounted upside down, about an inch from the top of the windscreen and right against the mirror housing. So far this worked for me in a few different toll booths on the NYS Thruway. Upside down appears to be key as mentioned above. YMMV
