I don't see a referral link field, just has my contact info and CC info..

You have to have clicked on a referral link which will take you to the main tesla site to order, and then you just follow through from there. Be warned, if you do not do so they will say they will not apply the code after the fact and that you have to cancel and re-order

Yeah, you'll have to back out and click a referral link and start over.
