I don't think you need to provide a formal driving history. Just describe in your own words what type of driving you do.

But it might all be a moot point anyway since Elon later tweeted that they plan to add a download button to the service setting in our cars in about 10 days to let people download FSD Beta on their own. So we may get FSD Beta without applying.
