This is for tax year 2015 but I believe it will also be in effect for this year.

For some people, perhaps. In my case, the fact I am subject to AMT made the value of the credit zero for me. I don't have the details, it was handled by my tax accountant.

I do not recall the Code mentioning that the EVSE had to be specifically of a certain type or make or configuration. Like with many things, if one would be examined, this would be a facts-and-circumstances hurdle to surmount.

I don't see a PDF with a form that I can use to submit a rebate on that page.
