Model 3 brake system is not responding causing bumper-to-bumper accident slow speed

Brake related issues may have a technical explanation such as this:

1). People say they felt the brakes go hard or not work without a lot of effort. But then worked later. That implies a temporary glitch. There are random reports.
I discovered suddenly that the power brakes were not working.

2). Tesla uses a brake booster that can be software controlled to adjust/reduce boost. This is done to achieve best braking performance between regen and hydraulic / manual braking. I imagine software could eliminate boost entirely thus leaving the driver with no hydraulic assistance and a very hard pedal.
For example the iBooster, used on all Teslas.
The iBooster’s ability to adjust the supporting force at any time in line with the hydraulic conditions by means of software controls.

3). Well, any crappy software result that we've ever seen from Tesla. Bugs, intermittent glitches, poor function.

I certainly don't discount the possibility that there could be braking-related issues that remain undiscovered.
