What tyre pressure do you run with 20s? I have just looked and both rear are at 43, nsf 42 and osf 45, I thought they should all be 45?

42 cold is generally what's recommended but a lot of the cars were delivered with 39-40 cold in them. Personally I think you could go a bit lower even than 39 cold but you might find range suffers very slightly.

When warm you should see around +2 or +3 higher than cold.

How long would you say it takes for the tyres to warm up?

Depends on your driving style, type of driving, conditions etc. If you're pottering around town at 20-25mph max. in the rain you may not see the pressures rise much at all. On a dry, twisty A-road in summer with some tight bends, you might see the pressures go up 2psi in 5-10 minutes. There's no definitive answer.
