42 cold is generally what's recommended but a lot of the cars were delivered with 39-40 cold in them. Personally I think you could go a bit lower even than 39 cold but you might find range suffers very slightly.
When warm you should see around +2 or +3 higher than cold.

Depends on your driving style, type of driving, conditions etc. If you're pottering around town at 20-25mph max. in the rain you may not see the pressures rise much at all. On a dry, twisty A-road in summer with some tight bends, you might see the pressures go up 2psi in 5-10 minutes. There's no definitive answer.
