You drove the car after adjusting the tire pressure, right? The car has to be driven a few miles to update the tire pressure. The app only reports what the car saw the last time it was driven.
You cant use either one of these (car or app) as live update numbers to fill your tires or anything.
Tesla's Model S TPMS system has a built-in feature that automatically detects a new set of wheel sensors. The TPMS sensors can be reset via the vehicle's touchscreen.
I had the same problem after rotating my tires. The car did apparently relearn the TPM positions but incorrectly. I had to take the car to the service center and they recalibrated the system. The service center did advise that the car updated incorrectly, that they have no idea why and that the only way to reset them was at the service center.
