This is a Tesla high output 40 amp chargers. Tesla's sells now sells different Gen 2 charger which only has a max output of 32 amps.
Works perfectly. Never had an issue.
Cosmetically, charging unit's silver cover has been removed and replaced with red sticker. Charger handle has marks/dents (see pic).

1 x NEMA 14-50 adapter (high output outlet, 240v / 50 amp) - Level 2
1 x NEMA 5-15 adapter (standard house outlet, 120v / 15a) - Level 1
