Having just driven the highland, I think you're OK to go with 19" wheels and not worry about the ride quality tradeoff. Just my two cents. The highland I drove had 18's, but the ride quality was night and day compared to my 2019 on 18's. even with comfort-tuned shocks.
Gear shift was a non-issue for me. Easy to do on the far left side of the screen. When you come to a stop, the gear shifter automatically pops up. I'm sure it would take a little getting used to, but no big deal.

@tivoboy It's probably your adjustable shoulder seatbelt mount rattling around. Easy test to just place your hand on the plastic seatbelt adjuster plate while it's rattling. If it shuts up, that's your culprit. Also pretty easy to pry off the B-pillar cover and stuff foam or something in...

Yep, the HVAC will make random noises as the heater warms up. It will also sometimes make these noises when the heat is set relative to outside temperature such that it can't quite balance whether it should be heating or cooling.
