In my semi logical mind...it would seem that the plastic bolt and nut would..should be reversed...Bolt head seen in the wheel well, not the threaded part and nut. Then ..why would anyone get that wrong in the installation process?
MY semi logical mind sees design/installation as a no brainer, the way it is designed. Wheelwell liner slides ONTO the threaded part, assuring proper alignment, w/o having to hunt blindly for a hole behind the liner. There's nothing "wrong" with the design, or the installation, other then improperly torqued nuts.
I will frequently reach in and check the nuts, to assure they're still there.
