I just wanted to follow up and say thanks again to everyone who responded. Tesla Mobile Service came out and replaced my charge port door today and now it works every time from almost 3ft away! Plus they let me keep the old door in case I break it (since it still technically works).

Thanks for the info! Yeah I definitely figured it was either broken superchargers or user error, but it just seems so consistent across chargers that it feels like a software issue. For example, I found that it takes about the same amount of time before the door eventually opens on its own...
