Update...I spoke with the company that makes the Juicebox. They recommended either the Hubbel or Pass and Seymour for a replacement outlet. I could only find the Pass and Seymour in my area so I replaced it with that. I also opened up a claim with Leviton and they want me to mail back the faulty...

It's only $29 (plus shipping) to get a placement plug for the Juicebox. No point in trying to be cheap and use the damaged one and hope it works. I will just have to use the 120v outlet with MC and our local supercharger until I can get it all fixed. Thanks again for everyone's feedback.
