I also have a 2015 Model S with the same issue. Both USB ports do not work but the 12V accessory does. First one USB port went, then the other went about a week later. Like the OP I wanted to use mine for phone charging.

It's fixed! A factory reset solved the issue. Many thanks to all who helped with special thanks to @tldickerson for the factory reset suggestion!
