CVRP rebate question: Form 4506-T

Only my name is on the purchase agreement and registration. But my wife's name is on our joint tax return. Do I still need to add her to Form 4506-T?

I did not have to submit the form. The process was pretty easy as long as you have your documents downloaded already. Should have done this the day after I took delivery.

My situation with the CVRP was a little more complicated in that I was re-assigning my personalized plate from an old vehicle to the 3. This is what I filed:

CVRP form with VIN and requested amount ($2500)
MVPA form
DMV new vehicle registration form (REG 397)
DMV vehicle transfer form (REG 262)
DMV special interest license plate form (REG 17)
Color scan of my drivers license

In my case it wasn't necessary to file the 4506-T (income verification). A PDF listing the CVRP supporting documentation is available here.
