Model S showing 3-phase charging on 120V UMC? Huh?

For some reason lately, my 2017 S75D is showing the 3-phase charging indication when I charge using my UMC on a 120V outlet at home. I know it's not the UMC since I just recently had it replaced, and the problem is persisting. I'm assuming something is busted in my charging inlet, or the on-board charger. Has anyone else ever experienced this?

Moderator note (bmah): Merged from a different thread. Since this topic has come up in a number of places there might be some opportunity for further consolidation of threads.

While charging this morning a circle with a 3 appeared on the charging screen between the added miles and amps. I’ve never seen it before but I’m getting a slightly higher charging speed. FYI it’s just a regular plug at a train station I park at everyday. Usually I get about 4mi/hr. I’m now getting 7. I use the chargers there once or twice a week and have never seen this.
