iPhone volume changes suddenly

Sometimes while I’m listening to a podcast from iPhone via Bluetooth, the volume suddenly goes down. To fix it, I turn the phone volume all the way down and then back up again. Normally when I adjust volume on the steering wheel, the phone volume slider reacts accordingly. But in this state, that does not happen. It seems like the two have fallen out of sync.
This started happening roughly 2 weeks ago, which is around the same time I upgraded from iPhone 12 to 14. I’m not sure if this is a bug in iOS or Tesla.
So far I have only noticed it with the Apple podcast app.
I’m currently on 11.1 (2023.12.9) and iOS 16.5.

Its the phone. I've seen it happen before and not in a Tesla. It's supposed to prevent listening to the volume too loud for long periods of time.
