Massachusetts EV rebate application question

Hello.
I just picked my M3LR last Tuesday. With the $3750 price drop, it qualifies for the Massachusetts EV rebate. My application got rejected because I didn’t include all the documentation. Has anyone in Mass gotten the rebate, and exactly did you have to submit? Thanks

I've gotten the MA rebate. I submitted exactly the documentation they asked for at mor-ev.org.

I've received two of them, just did what the website asked for.

Forgot about the state rebate. Was just just getting over the price drop and Fed tax credit. FUUUUUCK! LOL
