The card, as I recall, is free, but you will associate the card with one of your credit cards or bank accounts in order to 'reload' funds on the Flo card as needed.

I second the Kal Tire suggestion. The place in Milton does a ton of Teslas (and have no problem mounting Michelin XI3s on aftermarket rims), but there are a couple of locations in Hamilton, if Milton is a bit far afield for you.
The Ikea chargers in Burlington are notoriously problematic. The Chademo chargers in particular have been non-operative in the past. I suspect the problem lies with the charger and not your adaptor. Have you tried it at other chargers in the area to see if the problem repeats?

I used JML earlier this year and was quite satisfied. He's done a LOT of Tesla installs in the area and is quite familiar with the process. Keep in mind that the application for the rebate has to be made within 6 months of PURCHASE, not installation.

I had Opticoat applied to my White S85 and followed up by having Proskin applied to the hood, fenders, etc. I'm very pleased by the look of both. It cleans up very easily as well, and judging by the number of bugs that I wipe out every week, ease of cleaning is important.
