Not quite. It states in the manual that max battery power will only stay on for 2 hours. If you get out of your car for an hour and then come back to it, it will still be in max battery power mode or ludicrous+ mode (whatever you chose.) It will also not engage if it is below 20% state of charge.

Ludicrous+ mode is only enabled when using the easter egg icon, or from holding down the ludicrous button for 5 seconds and getting the light speed animation. You can tell them apart because the ludicrous+ mode enables the battery temperature display on the instrument cluster, and when it gets to optimal temperature (about 52 degrees C) the cluster changes again to display the transparent vehicle showing animations of both motors, and the left side of the cluster shows you the performance information of acceleration, battery power etc.
That's correct, as ludicrous+ is max battery mode and more finely tuned power release. They released it some time after as an easter egg feature instead of as part of the general options.

No, Ludicrous Plus implies max battery automatically. There is no ludicrous plus without max battery. 1 and 2 are the same thing. Otherwise your list is correct.
Launch mode is only available if either ludicrous+ mode or max battery mode is enabled, and only if the ambient temperature is above 4 degrees and state of charge above 20%. The battery temperature doesn't need to be up to optimal yet though.
Time to heat up depends on ambient temperature, current battery temperature, and use of battery. In summer I get it within 10 mins, in winter if I enable it while it's in the garage and not driving, it can take over 1.5hrs.
