In my case it was in the mail. I had home delivery so I got an odometer statement, MVPA, filled in title application, and a certificate of origin. Maybe you already have some of these. The MVPA in the packet only had the production complete date on it but they were able to use the odometer statement as the delivery date. If the delivery date is more than 30 days ago there are penalties and interest on the sales tax so be sure you have that documented somehow. You will also need your proof of insurance for the Tesla and registration for the car you traded in or sold so you get that credited back.
In my case they actually sent it via FedEx probably because it was one thing they could do to make up for all the other troubles I had with delivery. So if anyone is close to 30 days without their forms tell them to FedEx it so you avoid penalties.
